Mental Health 

Skill Building

Mental Health Skill Building Services, are time-limited, training and supports designed to promote

successful community living, foster maximum independence and prevent psychiatric hospitalization through a continuum of support services. Mental Health Skill Building assists adults who suffer with mental illness to achieve and maintain community stability through teaching and reinforcement of independent living skills, problem-solving and skill-building in activities of daily living, including transportation utilization, resource management, budgeting, educational and vocational activities, symptom recognition and management, social integration, medication management, and monitoring of physical health and nutrition.

The services and supports are provided to individual's in their homes and in their communities, and at a frequency based on individual need. In addition to an assigned Mental Health Support Specialist, who assists individual's in identifying and meeting their individual service goals; individual's may be supported in their recovery and community stabilization with a peer mentor. Peer mentors, individuals who are in recovery from a mental health condition, support others on their own paths to recovery by sharing their own experiences and providing assistance to individuals to create their own wellness recovery plan.

SERVICES WE PROVIDE:

  • Clinical evaluation of mental health status to determine eligibility for services and individual service plan needs

  • Life Skill assessment to assess level of independent living skill functioning (during service initiation and on an interval basis)

  • Education and skill training or acquisition.

  • Crisis and Individual service planning to address specific supports and services to be provided and the goals and objectives to be accomplished.

  • Consumer Advocacy and partnership with persons/agencies who assist and support individuals with their community stability.

  • Post Discharge Follow-up to include telephone contact for a period of up 30 days to ensure client stability and continued access to vital resources.

CRITERIA

FOR ADMISSION

* Individual must have a diagnosis of mental illness and a documented prior history of qualifying mental health treatment. Qualifying mental health treatment is considered as at least one of the following: psychiatric hospitalization, residential treatment, residential crisis stabilization, PACT or ICT services, RTC-Level C placement, or A TDO evaluation due to mental health decompensation


* The individual's mental illness results in functional limitations in major life activities and the individual requires individualized training in order to achieve or maintain independent living in the community. Individualized training may be required in the area of acquiring basic living skills such as symptom management; adherence to psychiatric and medication treatment plans; development and appropriate use of social skills and personal support system; personal hygiene; food preparation; or money management.
